3. Personalized Recommendations

  • Develop practical solutions tailored to the individual.

    • Recommendations may include home modifications, accessibility improvements, adaptive equipment, safety strategies, caregiver education, and future planning considerations.

4. Follow-Up & Collaboration

  • Support implementation and ongoing planning.
